Many families obtain a referral from their GP or Paediatrician, under the Medicare Mental Health Care Plan scheme. When your referring doctor completes the plan, they will normally fax it to me (Simon Beal Fax No: 84503385), or sometimes they will give the referral directly to you and you will then need to send it via fax or scan it and email it prior to your first appointment.
(Please note: a referral is not essential to make an appointment. A Mental Health Care Plan referral is required if you wish to claim the Medicare rebate and to be bulk-billed.)
When a referral is received, I will send a text message notifying that I have received your referral and you are welcome to visit my website to check appointment times and make a time to suit. If you have any questions about the referral or how best to proceed, you are welcome to contact me by email. If you have been referred and haven’t received a text message from me within a few days, please let me know by email, as it is possible I haven’t received your referral.
When you book an appointment online via my website, you will receive a email confirmation and an intake form, which allows you to provide some details prior to your appointment. If you wish to provide more information, you are welcome to email this directly to me prior to your appointment.
Please feel free to email me with any questions or concerns, or Ph: 0457 365 857 and leave a message.
